Attached is an electronic claim form to enable you to lodge your claim it is viewable using Adobe Reader version 7 plus. This is an Electronic form which needs to be completed online and is not to be printed and then hand written. Should you prefer a form to hand write please contact the office and an alternative will be sent.

When you have completed the form and all mandatory fields have been completed please click the Submit button at the end of the form, you will then receive an email screen which will require you to select Send. To ensure your email containing details of the claim has been sent, please refer to your email Sent Items folder. Once Swann Insurance receives your claim form we will reply to you confirming receipt within 48 business hours. If you do not receive this email please contact the office.

It is important that all questions are correctly and fully answered by the policy holder. This will enable Swann Insurance to proceed with the processing of your claim; delays could occur if the claim is completed by someone other than the policy holder or if insufficient information is supplied. If for some reason the policy holder is unable to complete this form please contact the office to discuss options.
Acceptance of this form by Swann Insurance does not constitute authorisation of the claim at this stage.
Please forward a copy of the quote from your preferred repairer. If your motor vehicle/motorcycle is a Total Loss or a possible Total Loss, please also forward a copy of your motor vehicle/motorcycle purchase documents and your current registration certificate. Any correspondence or demands received from another party involved should also be forwarded. These documents can be forwarded by the following methods and ensure your policy number is clearly noted on the documentation:
